We are very excited to have a new Panther in the Family! Follow the instructions below to enroll a new Cristo Rey Tampa student after you have received your acceptance letter.
Please call the office to make an Enrollment Appointment for Admissions and Corporate Work Study Program (813-621-8300).
Parent /Guardian and Student must be present to complete the process.
Requirements for Admissions Enrollment:
Enrollment Fee: $75 due at the time of enrollment for all students. We accept checks and money orders for the exact amount payable to Cristo Rey Tampa High School.
Step Up for Students Scholarship (SUFS): All Cristo Rey Tampa students must qualify for the SUFS income-based scholarship.
- Please bring your SUFS Award Letter at the time of enrollment.
- For new applicants, you will need to submit a copy once your child has been approved. Enrollment is conditional until that time.
Enrollment Contract: Fill out both sides of the enclosed enrollment form.
Smart Tuition: Complete and submit Smart Tuition plan form. Every parent must complete the required information for their family account through Smart Tuition, our tuition payment provider. Multiple students are included in one family account. It is mandatory to complete the form whether you are making full, semiannual, or monthly payments.

Important: All students attending Cristo Rey Tampa High School must be in full compliance and wearing their uniform by their first day of Summer Bridge or School whichever comes first.
PSAT 8/9 Testing: All incoming freshmen and transfer students are required to take the PSAT 8/9. The test provides accurate information of a student’s reading, writing and math skills. This information will be used to accurately place students into classes. You can register for one of the testing dates listed below by sending an email to firstname.lastname@example.org with the date selected, student name and contact information.
- Thursday, April 6, 4:30 p.m. (check-in opens at 4:15 p.m.)
- Saturday, April 8, 8:30 a.m. (check-in opens at 8 a.m.)
Enrollment Health Requirements: In accordance with Florida State Law, all incoming students are required to present a Florida Certification of Immunizations (DH Form 680) which must include 7th grade shots and DOE Code 8 must be met. In addition, all incoming students must present a School Entry Health Exam (DH Form 3040) which must be current within one year or after August 1, 2016, and Health form page 1 must be signed by parent and page 2 must be signed by doctor. Both forms must be originals and are available through your family doctor or the nearest health department facility.
Important: Students cannot start school without these forms on file.
Sports Physicals and mandatory online FHSAA course: The sports physical is valid for 365 days from the date indicated on the form. Students must be enrolled (except for middle school final records) and have FHSAA physical and consent forms on file before they can participate in any athletic activity, including conditioning and tryouts.
Important: Please note that the Sports Physical and the School Entry Health Exam are two different forms. We recommend that all students get both done at the same doctor’s office visit.
These are the required sports physical forms:
- FHSAA Preparticipation Physical Evaluation (form EL2 contains 3 pages)
- FHSAA Consent and Release from Liability Certificate (form EL3 contains 4 pages)
Physical forms EL2 and EL3 can be downloaded at www.fhsaa.org once at home page look for the forms under the menu bar choice for Students and Parents.
Mandatory FHSAA online course:
FHSAA requires that all student athletes complete an online course titled “Concussion in Sports”. This course is mandatory for all FFHSAA member school athletes. The free course is offered at the National Federation of State High School Associations’ education website: www.nfhslearn.com . Every student-athlete must create an account, select the course “Concussion in Sports” and print out the certificate of completion. The certificate of completion must be turned in to the Admissions Office before you can tryout, train or practice. No exceptions.
CRT Athletic Forms: In addition to the FHSAA forms and online course, all players and parents will complete and submit the following forms prior to any student trying out, training, practicing, or playing in any sport:
- Rules and Conduct for Athletics
- Statement of Compliance
- Athletic Events Consent and Release
Requirements for Corporate Work Study Program (CWSP) Enrollment:
Students are required to participate in the Corporate Work Study Program (CWSP) to be enrolled and registered as a student at Cristo Rey Tampa High School. Parents/Guardians and the Student are required to complete and submit employment forms to the CWSP Office at the time of enrollment.
Important: In accordance with the U.S. Department of Labor, all students enrolled at Cristo Rey Tampa High School must be at least 14 years of age by September 1 of their freshman year.
I-9 Form (Federal Employment Eligibility Form)
- Student must be present when I-9 is completed.
- Student must complete the I-9 form prior to beginning work.
- See attached list of documentation that is required to officially verify citizenship. You must bring ORIGINAL DOCUMENTS to Cristo Rey Tampa at time of enrollment. Student must submit either one document from column A or two documents (one from column B and one from column C) to the CWSP Office when you enroll.
- I-9 form will be completed in person at enrollment upon presentation of original documentation.
